INSTRUCTION
This teacher evaluation resource is intended to selected participants.
Yes YOU! You are one of the selected participants to do the teacher evaluation mock-up. This Teacher Evaluation resource will direct you to a link or form upon clicking. But before you proceed, please read the following carefully.
The teacher evaluation form has 3 parts: (1) course/subject profile, (2) the teacher evaluation matrix, and (3) your evaluation of the tool.
In the first part, you will be asked about the name of your teacher, course or subject taught by the teacher, semester, and school year.
In the 2nd part, you will be asked to click the button corresponding your evaluation to your teacher. POOR as the lowest, OUTSTANDING as the highest. A common comment or suggestion about the teacher's teaching performance is optional.
The 3rd part will ask your honest evaluation of the tool used in the 2nd part. AGREE TO A VERY GREAT EXTENT as the highest, DISAGREE TO A VERY GREAT EXTENT as the lowest.
You need to fill out all required items or questions. You may opt not to fill out not required items.
Click the next button to proceed to the 2nd and 3rd part of the form. Note that you cannot proceed to the next part if you fail to provide responses on required items.
